When doctors fax a patient referral to (310) 301-5391, the physician referral representatives will call your patient the same day or by the following morning to coordinate a referral.

Hospital-to-Hospital Transfer
The UCLA Transfer Center (310-825-0909) may be able to further assist in facilitating transfers to 皇冠hga025大学洛杉矶分校健康 hospitals.
Our goal is to help patients and surrounding referring facilities when transfer requests are initiated. 这样做, we are asking to have all essential patient information collected and readily available when a transfer request is initiated. Please download our transfer packet below and be prepared to answer the questions and fax information from this packet.
